Ampersand Health are excited to be working in partnership with St George’s University Hospitals NHS Foundation Trust, Thriva Health and Firefly Scientific Limited to collaboratively implement new patient pathways, underpinned by our integrated digital technologies within the Inflammatory Bowel Disease (IBD) service at the Trust.
As the name suggests, the Digital Health Partnership Award was created to highlight how a partnership approach can help remove the barriers that often exist when trying to implement digital solutions within the NHS.
Ampersand Health’s platform, integrated with the CalproSmart™ at home test for faecal calprotectin and Thriva’s home blood testing, will enable the IBD team at St George’s Hospitals to enhance the care provided to patients and address elective recovery challenges, through:
– Deploying Ampersand Health’s condition specific mobile app (My IBD Care) to support patients with self-management
– Utilising Ampersand Health’s integrated cloud-based clinician portal, integrated with the at home testing providers, to remotely monitor and asynchronously communicate with patients
– Transitioning specific cohorts of IBD patients from current lab-based testing to at-home testing, for both faecal calprotectin and blood tests, including safety related drug monitoring
– Implementing patient-initiated follow-up pathways (#PIFU)
